Capture the service-useful facts. This is documentation, not a teardown checklist. Door opening is stable enough for observation No visible off-track condition, damaged spring/cable, or uncontrolled door movement. Opener has normal power Outlet/opener lights or other normal power evidence are present. Wall control response recorded Note whether it opens, closes, requires constant pressure, or does nothing. Remote/keypad behavior recorded Separate transmitter behavior from the wall control. Both safety-sensor LEDs recorded Capture color plus solid / flicker / off instead of only writing “sensors look okay.” Sensor beam path checked visually Record obstruction, dirt, bracket movement, sunlight or a clear path without defeating the safety system. Exact code / blink pattern recorded Count the full repeated pattern and note which LED or arrow produced it. Reversal / stop point recorded Before the floor, at floor contact, while opening, or no travel at all.

STOP DIY / HAND OFF Do not use the worksheet as permission to keep cycling an unsafe door. Stop if the door is crooked or off-track, a spring/cable/roller/track component is visibly damaged, the door is unusually heavy or uncontrolled, or opener wiring/terminals show heat or electrical damage.
